The CREATE Health Passport is a communication tool designed to support people with disabilities express their needs when in a health care setting.
It also assists and enables healthcare staff to provide care by providing reasonable adjustments before undertaking any assessment, examination or treatment of individuals with disabilities.
The St Michael's House CREATE Health Passport Project team led by Feabhra Mullally, recently hosted focus groups from our day services, ASD services, residential services and community support services. These focus groups trialled the new HSE Health Passport App and subsequently provided valuable insights and feedback towards creating a tool that supports better health management for everyone. Their time and contributions are very much appreciated as we work together to improve healthcare accessiblity and efficiency.
